Once you beat a scenario, you get a title. There are two seperate titles I think for both Warden and Prisioner difficulty. When you complete a scenario You unlock its carryover effects to use in a normal run. Also you can equip titles you earned in any run as well.
These descensions are there to spice things up and usually makes things harder. Good stuff for end-game challenge. I enjoyed grinding them. But I am a casual player so I used several mods to adjust difficulty to my liking.
