Inmates should ejaculate several times on a floor, not on Karryn. Saw an advice here to go for pure build, reach Floor 2, start the fight with Nerds, kill everything that isn't nerds. Maybe use Flaunt, if someone is not agitated enough. They will stay back masturbating until the very end.

I got it randomly just in normal sloot battle interaction.

Make sure you set the options to disable footjobs and rimjobs. It makes some achievements (like this one) easier.

Or since you'll be in a pure build - just keep related desires in check, so nerds won't be able to initiate anything on you, forcing them to stay behind and just masturbate.

I've also done this against the guards - I've found early on, they're reasonably trivial to get going instead of attacking you,
