2) No, they don't. Your base stat value affects that, the more STR you have - the harder it will be to level it up. Using Edicts giving +30% to STR leveling for example - is a great way to control which stats you'd like to focus on.
Some items just give points. Think of them like armor. You unequip, it goes away.
Some items give growth. That's very important to have as early as possible as it makes your focus more stronger. Growth items of course do nothing once you hit the level cap.
Thus, if you're playing on a harder difficulty, or with a tougher challenge run on, focusing on the stats you WANT is critically important. Want to play a dex/agi build? DO NOT use strength attacks so you don't gain strength levels. Don't kick people out of the bar if playing waitress. Keep your desires low so you don't gain endurance levels. Avoid using energy moves whenever possible. All so that you gain a minimum of xp in the stats you aren't focused on. It's a "learn by doing" system, and ALSO a "you have x number of skillups and no reset button" system, so to get a strong build, you need to only DO the things you want to do, and avoid doing anything else.
on low difficulties or with easy builds, it doesn't really matter. you can probably faceroll your way to success on secretary mode fairly easily with only a mediocre knowledge of the game. But if you're running "A Disappointment" challenge in prisoner mode? you'd better carefully ration every single skillup.
Those edicts and some accessories are super helpful for that. They can either increase the rate at which you gain a stat's xp, or reduce the rate at which you gain unwanted stat xp.
