Story is the exact same, no changes, no hidden ending (thank god!).
Gameplay is mostly the same.
They added a really unnecessary and grindy Buster Sword level up system that you can mostly ignore.
The Buster Sword looks different now to be the same model as the one in the FF7 Remake.
The voice actors have been changed to be the ones from the FF7 Remake (some for the worse *cough* Zack *cough*)
They changed some (voice)lines (ex: Cloud doesn't say "I've never seen you use that sword" in reference to what Zack said to Angeal but "I really like that sword!") and actually ♥♥♥♥♥♥ up some existing ones (ex: Cloud's DMW memory during the final fight says his line twice for some reason, funnily enough with the exact voiceline I mentioned before).
Visuals have been updated obviously, HOWEVER, all models still use the PSP animation skeletons (which can look incredibly goofy).
They added a hint system (that outright tells you what to do and doesn't HINT) for things like how to obtain the Magic Pot materia for example.
They included a weird slideshow montage after Angeal's death showing Zack hang out with the Turks (might have been in some versions of the OG, I'm from Europe so I played that version).
The way you execute spells/skills is different now (You don't scroll through a menu but press assigned hotkeys)
You have extended options in the settings menu like audio sliders (which don't work for most cutscenes as they mute the entire audio instead of only the music) and obviously graphics settings.
It has achievements.
Edit 3: The game also now features a so called "combat evaluation" after every encounter, ranking you on how well/efficiently you cleared it. This gives you HP/MP restore and MASSIVELY cuts back on menuing to use potions etc.
There might be more I'm forgetting but these are the big ones that came to mind. All in all it is a good (though not excellent) remake with some major downsides that could ruin your experience depending on how much you care about certain things like the voice acting.
Edit: Things like the voice acting and the new Buster Sword model can be easily reverted (or rather: fixed) with mods btw.
Edit 2: Sorry, forgot to mention the performance. It ran incredibly smooth at max settings, 60 FPS with no dips below maybe 50 on both my older machine as well as my new one. So if you meet the recommended specs on the store page you should be good.
