Baffled With The Relationships At The Start

I don't understand the relationships the main character has with the other women in the house. One woman calls him "sweetie" and makes him breakfast. Is she his mom? Girlfriend? How are we supposed to know what relationships are going to make sense in this game?

Yes, the women in the household are supposed to be his mother and two sisters. The game's development is mainly funded through Patreon and 2-3 years ago they had a terms of service change banning a bunch of content including incest and hypnosis. The text entry options upon starting a new game where the players enters the PC's relations to the various characters is supposed to be the workaround.

Well, I always, preferred to think, Elisabeth is his stepmother in the worst case. It’s too disgusting otherwise, and spoils all pleasure from the game. The protagonist never calls her mom, right? Just teacher. And she thinks about him as about her pupil, not as her son. So I don’t understand why I should think otherwise. My guess, she is really just his teacher who lets him and his relatives live in her house. Why? I don’t know why. Maybe she knew his parents and they asked her to shelter him and his relatives while they would be far far away. Maybe they just somehow mysteriously disappeared and she let him and his relatives live in her house so she could look after them. That’s it.
And Megan is his cousin, not sister.

I went with aunt, but in the way that she's his late uncle's wife, not blood related. Jess and Megan I just listed as her nieces, or the player character's third cousins, twice removed.

I decided that the protagonist's parents had to go out of town and the protagonist is staying with a neighbor and her daughters.
