Voice of El related stuff is nearly always completely out of the hand of the modding tools, there is nothing a mod could do to change how the event works. I mean... well, you COULD create a counter event, to heal every unit everywhere from afflictions every turn, maybe, but that would probably cause more harm than good, balance-wise.
But yeah, once the Apocalypse started, it is all down hill from there. That is... like... the Apocalypse. You may not even have seen the worst of the worst from it, if it just freshly happened.
The more seals the Voice of El player breaks, the worse stuff happens. I think they also cause forest fires for example.
Though forest fires can also occure as normal event.
I heard rumours about a seperate War in the Heavens event, where Celestials and Infernals duke it out in Elysium regardless of these circumstances, but I never saw that, so it might be a misunderstanding on my part.
Then there is a rare item, which itself is called a Heavenly Seal, which upon activation also breaks a Seal and can further the Apocalypse. But no idea, if the AI would use such an item, or even how you could find it.
All of these circumstances would break the Seal, like an El player does it, including the disease part. I think that covers all of the possibilities.
Edit: But yeah, the very most likely reason is just an Voice of El player/AI.
I only play maps generated with the editor (100x100), so I guess the era does not matter?
The demons will usually go for settlements to harvest Sinners. While they will attack enemies on their way, they usually don't really go for citadels or mines.
They also don't really seem to fly, so if you have a foothold in the Sky or Agartha that isn't reachable on foot, you should be safe.
Of course to win you will have to close the gate.
Only citadels in the Sky or Agartha count for keeping you in the game however. Even if you have a citadel in the Elemental Plane of Earth, that will not safe you from losing.
If you are careful and quick you can also sneak in and close the gate. Stealthy armies are good for that, as are fast ones or flying ones that can airdrop next to the gate.
BATTLEMODE is currently playing a monkey game where he managed to close the Demon Gate. Twice.

Still, the Sky should be somewhat safe. Getting on an island with a port might also work.
Interesting. Thanks for the info. I haven't had that event happening to me. I did remember someone mentioning that they forted up on an island as Voice of El to wait out the demise of their last AI-enemy.
