Am I missing something or does the game keep running while I'm offline?

I really, really dislike anything similar to "dailies", am I missing something or am I effectively forced to log in every couple days so my castle heart doesn't decay?

At the very first level, you will need to log on in 2 days to refill it. But frankly, you should be beyond the first level of a heart in 2-3 hours of playtime. From there it quickly increases, capping off at ~2 weeks on officials.

If you play on a dedicated server, you'll need to log in regularly to keep your castle from decaying and deleting itself.

If you play on your own private server. time won't pass for your castle heart while you're offline, so you'll be safe to leave and return as you desire.

What's the max I can top it off of and can I realistically get there quickly? I mostly only game on the weekend.

Depends heavily on server settings. Blood essence consumption could be reduced, stack size increased. The first level or two for your castle wouldn't take very long to upgrade, but that would only give you a few days, maybe a little over a week at default settings to make sure you return to refill the heart.

If you are not confident in your ability to regularly return to fill your castle heart, I highly recommend you do not play on a dedicated server.

If you're only able to play on weekends, you can still play on dedicated servers, just keep in mind that you need to keep your heart filled with essence to maintain it. You also need to be wary of servers with castle raiding times, because during such times, if someone desires it and has the appropriate "tools", they are able to break in and destroy your castle if they so desire, and you may not be necessarily around to defend it.

That being said, there's a lot of different servers hosted out there, some of which may have disabled castle raiding entirely, or only enable it on the weekends when some people are actually able to freely be on to contest it, like yourself.

This game has a lot of customization for settings, you just need to find a server that suits your needs.

Solo, or with friends only. Could try the LFG subforums here, or joining the V Rising Discord channel to find others to play with.

Suzaku is right it does not if the settings are set right on the server also Nox some dedicated servers players make have really long castle blood drain rate mine included i have a level 5 castle heart on my dedicated and it has 200 days of blood in it. you can in fact have a long decay rate and drain rate i also have my resources set to X4

PS my server is PvE and castle raid and castle damage is set to never

Yeah if its dedicated like others said you got to. once you upgrade the heart you can increase it I am level 3 heart I got 8+ days now on my castle heart I can vanish but on private not sure if that is a server setting or default but feels like its on default to me..
