It's kind of a get good thing as well but mostly just learning the boss mechanics and trying to adapt your play-style... Maybe try different abilities and/or weapons.

Are you on easy, normal or brutal?
1. Kill Beatrice the Tailor to unlock the loom and new tailoring station to make your base Hollowfang set (uses wool thread and cotton yarn).
2. Kill Leandra the Shadow Priestess to unlock the amulet crafting station to make your base scourgestone necklace (uses gem dust and scourgestones).
3. At this point you'll want access to the merciless upgrade to your base iron weapon.
4. The next progression path is locked behind killing Maja in her tower. She's mostly a DPS check in the sense that you absolutely have to kill the minion she shields and pulls into her book. If you don't she's capable of healing almost to full, provided you're playing on brutal.
5. Track and kill Bane the Shadowblade, who's roaming Dunley hidden under a cloak, and get the ability to transform into a human.
6. This allows you to trade silver coins to the book vendor at the Dunley farmer's market. Although merciless iron books can drop in the open world this is a more reliable way to get what you're looking for. It'll also allow you to pick up the new set of upgrades for your armour. Note that the shop stock rotates hourly.
7. To actually build your merciless iron weapon and the your next set of armour you'll need to kill three bosses.
8. For the weapon you'll need (a) reinforced planks (iron + planks) from killing Vincent the Frostbringer, another roaming boss in Dunley, and (b) concentrated blood essence or whatever it's called, which can be produced either from unsullied heart at the blood press or by killing Tristan the Vampire Hunter in Farbane and converting 150 blood essence into 1 unit of concentrated blood.
9. Finally for your amulet upgrade you'll need to kill Grethel the Glassblower in the Quarry in Dunley so you can produce sheet of glass.
Taken together this will get you ready to challenge the final area boss in Dunley and move on to Chapter 3.

Weapons have a normal type and a "merciless" type for +3 lvl.
Armor has 4 separate pieces to upgrade, for +1 lvl each and probably a +1 set bonus. This is the slowest one, because it means unlocking and shopping for recipes.
The amulet has an upgraded variant for +3 lvl.
Eventually you'll find magic weapons which give +2 lvl for the first one.
I think in brutal, all V Bosses have +3 levels. (I've only played brutal, so I haven't actually confirmed. But that's what the game says when you're choosing difficulty). So I think it becomes more common in brutal to fight V Bosses that are higher level than you.
That said, I think all the boss fights can be pretty easy if you play conservatively. Just learn the boss's mechanics, use 2 defensive spells, then cycle through your defense spells + dodge to avoid all of the boss's attacks while poking them down at range.
