You pick it based on which saddle you make in the leather-working bench once you unlock the undead mount at castle heart lvl 4.
I got the DLC for a friend of mine that is, and who always griped about the in-game castles not being "cool" enough to be a Dread Vampire Lord's Castle.
He no longer has anything bad to say about this game.

Like all cosmetics, buy the cosmetics if you want to support the creators.
Do bear in mind that, if you play in a group, you are able to use other people's skins or they yours as long as you ask them to give them over or you use the decorations and they move them themselves, so if you generally play with friends, you can each get one of the dlcs and have all the skins.
Personally, I have all DLCs because I really like the game, but you can play the game normally just fine without so if you feel it's pricey, screw it...Hahaha
