Is there a way to customize difficulty or just the 3 presets?

If we're talking about combat difficult and in game, then the short answer is NO. When they added those presets, they also removed the ability to tune enemies stats and levels individually.

However, I think you can tune some extra values by going to GameServerSettings.json file under AppData/Local Low/Stunlock Studios/. I was under a situations were Brutal were too hard and Normal too easy for me, so I edited that file to reduce VBlood Damage from 1.7 to 1.25 at (these are default Brutal settings):

"UnitStatModifiers_Global": { < Changes the stats of normal unities
"PowerModifier": 1.4 -- Damage (+40%)
},
"UnitStatModifiers_VBlood": { < Changes the stats of V-Blood
"MaxHealthModifier": 1.25, -- HP (+25%)
"PowerModifier": 1.7, -- Damage (+70%)
"LevelIncrease": 3 -- Extra levels above the usual (+3 above the ones on normal difficulty)
}

The above have something to do with normal unities at Brutal Difficult (+40% damage). The bellow is VBlood (+25% HP, +70% Damage and +3 Levels from normal difficulty).

You can also use these same settings to make things even worse, like increasing it even more. You can also add "MaxHealthModifier" to "UnitStatModifiers_Global" to increase or decrease fooder unities HP. You can check these made effect by checking the system screen in game, it'll tell what's changed if compared to Normal Mode.

If you have chosen Private Game then just beneath the presets is a button labelled Advanced Game Settings. That's where you choose your own difficulty.
The author of this thread has indicated that this post answers the original topic.
