Expertise is decided upon by a dice roll when you convert a servant (based on their blood % - a 100%'s best possible dice roll is 26% Expertise). Expertise adds an ADDITIONAL % of power that stacks up with better gear.
So long story short - If your Servant has an Expertise of 10%, they get an additional 10% of their power on top. This is primarily for missions, but it also adds additional health and damage to PvP (although not a ton)
Afraid not - The only loot bonus comes from having servants that match the region the mission is in (so no Cursed Forest bonuses possible)
Naw - The game just rerolls the expertise every time they die. So if the expertise can be 9-14% a death can result in any number in that range for better or worse.
