Chaos and Conflagration Bug!
As some of you know, there is a bug in this "Chaos and Conflagration" job, where you have the weapon maxed out, but it shows that you only got 3 of the 4 items needed to upgrade the weapon.

1 - Can anyone tell me if this is for everyone or just a few?

2 - Will this job be completed if I play on NG+?

You will get 3 upgrades for your blades by just progressing in the story. For the final upgrade, you have to visit Muspelheim and complete all 5 normal and hard trials. Once you do you will reach the top of the mountain where a Valkyrie named Gondul awaits.

Defeat her and you will obtain an item called Raging Inferno of Muspelheim. Use this in the shop to buy a Chaos Flame and fully upgrade your blades.

Defeat the Muspelheim Valkyrie. She drops the “Raging Inferno of Muspelheim” resource item.
Go to any shop > Buy > Resources > Chaos Flame. This exchanges the Raging Inferno for the item you need. Now in the shop go to Upgrade > Blades of Chaos

(you must have beaten the story to exchange the item, means you have to bring the ash to Jotunheim's highest peak first).

You dont need to finish the story to max out your weapons... I havent even beaten the story and I already have those maxed out. Every guide that says it can only be done after the story is downright wrong.

It just confuses the person asking the question when it is completely misleading. Not mentioning it at all would have been the thing to do.
