Lighthouse ladder is out so clearly you need to find another item to get you up there.
There's more gold keys in chapter 2 this is true.
Check for an Item called Black Raven Skull and there are 2 of them in Chapter 2.
There's a black raven skull in a stump out in the swamp. Usually around the starter area (although once found it out near the fort). It teleports you to Alvaro the flamboyantly pompous. The lighthouse needs no bulb as long as one as blazing as him burns within its tower.
There's a second raven skull that takes you to the Doctor's burned out cabin (each skull is keyed to a specific location but turns into a duplicate of one present on the current map if taken to a different map. The canyon one turns into the Doctor cabin one). The second is either on top a bookshelf in a side building next to the back entrance of the fort (it has a safe inside it and a dapper dead) or by parkour to the roof of a guard tower (I've found it on two different guard towers roofs, the stable roof, and only once in the storehouse office (the mentioned side building). It's supposed to be in the office).
It's for the "Alvaro's Friend" and "Alvaro's Best Friend" chievos. For the second achievement:
The "Alvaro's Best Friend" chievo can be gotten either with the skull gotten in the fort or the intended way. The intended way for it is to pick up the Chaos Die from the abandoned passenger car outside the first town in the mountains. Use the area looting interface, it's near impossible to see the thing normally. After that you need to roll for a "mysterious teleport". I highly highly HIGHLY recommend doing this at full health and with the Cultist Mask. You can use Purple Revival to reset the CD btw. The Cultist Mask turns you invisible while at full health and undetected. Especially given where the skull is.
Places I have been teleported, in order:
-Cultist cave overlooking the north hunter camp on the side with three hellbears doing an autobiographical nature documentary on themselves down in the forest with a quest item.
-Cave with miner overlooking the carriage with the item for Alvaro's achievement.
-Cave full of broken screams (invisible wendigo variant that are as numerous as birdmen were in the canyons) overlooking the part of the sawmill train depot that had two broken screams.
-And finally the raven skull cave. Which is the broken scream lair overlooking a forest full of broken screams down from a village made of broken screams where a quest item is which is outside a secret cave of broken screams. The forest in turn is on a cliff that overlooks environmental storytelling about broken screams. If you couldn't tell I really hate broken screams. Never died to one, they're just really annoying and basically a mini-boss monster treated like a trashmob. Anyway, only the ones in the lair are visible. You are likely to land on top of the black raven skull, in the same charnal pit (if the teleport take you to a place covered in blood and guts and body parts then you're at the right place). If you go for this and plan to fight your way out I recommend Call of the Abyss, one headshot puts them down nice and clean with Gunslinger and Gotcha.
Do note you NEED a second item for the achievement. If you get Alvaro's Rib then the Painting Cutthroat will appear stabbed into a crate in the Undead's camp. There's a carriage off next to a tunnel. Cut the painting's throat. Pick up Alvaro's Amulet. Take both to the talking horse.
