Hey, any suggestions on how to get these items?

I tried the bear head and shirt but it didn't work.

Unsure of all the combinations, but I know the Bean Head and Plant Suit will do it.

Do the Main / Side Missions and you will get the Plant Suit and the Bean Head.. Equip both on one. And you will get that Achievement
