There are a couple points where you make decisions that slightly altar some things in the game and I'll list those that I know, but ultimately it won't effect the overall ending of the game or how it's played. Specifically with Hoags death, the only difference will be that if he lives, you could find him at the speakeasy and his sister (the bartender) will give you a half-heart. There's a handful of interactions like that that promote the player going through the game multiple times. Hope this helps
You can choose to kill or spare Dreg.
You can choose whether to tattle on Brad.
Those are the main ones that I think can affect npcs.
I'm going to try to remember them all. I might be misrembering.
I got one from giving Sewert the gem.
One from the Mayor's old room.
One from saving the kid with the sprained ankle.
One from not tattling on Brad.
One from repairing the broken robot.
One from saving Oli.
One from the Wraith quest.
One from Barbara if you escape the mines with Hoag.
I didn't got the Hoag one but got one from the other prison lady when I found her latter in the city. But I think I had to talk to her twice. Hoag didn't survive with me. Not sure what other piece I missed because I ended up with just three extra health and one spare half-heart.
EDIT: I know now, I didn't tattled on Brad. So one piece less from that.
