What happens in 2nd and 3rd playthrough?

Because no way am I slogging through this boring game 2 more times. Does the story actually wrap up? Does Cath live? Does the Ruling get explained at all?

Yes to all. If it's any kind of incentive, the second playthrough is shorter than the first and the third is even shorter.

Absolutely worth it. And the other 2 playthroughs don't have much overlap with the first one, it's more a continuation. IMO, having the "acts" of the game disguised as playthroughs is a serious disservice to the game IMO. I've seen several reviews online where they clearly only experienced half the story.
