A) There's stuff in the only rune room I've looked at that makes me think it holds hints for another puzzle, so if that theory holds true there's at least one thing after that
B) I have a lead I can't follow yet that implies there's a whole 'nother adventure still waiting if that's the only thing you've done past room 46.
I can't say for sure, but I wouldn't be surprised if the credits roll when 75% of the content is still hidden beneath the surface. I've heard vague hints at all KINDS of puzzles that make me go "wait what the heck are these people talking about? Does this game ever end?"
The biggest unresolved mystery at the moment is regarding the 8th safe and red envelope. It *might* not even exist since Alzara says we will never open the last one, but it's still a bit hard to believe with 8 being such an important number.

What exactly do you mean by "if that's the only thing you've done" past room 46? What he's describing is many many many many hours / puzzles post-46
I mean, once you pull a sanctum key you kind've know what you're in for for that whole section. Yes, there is a LOT to do there, so that might sound confusing, but imagine a tree with two branches that each split into many many many branches. The many many smaller branches are the sanctum puzzles, but I believe there is another major branch you can also explore postgame. I haven't gotten to follow the lead yet, so I have only half a clue what I'm talking about and it should be taken with a grain of salt.
Given the effort to acquire keys and find the symbols, I believe in your analogy each sanctum key / sigil is a big branch and the steps to get them are the little branches
But there are also some other big branches, sure
