Question about open safes (spoilers)

Five or six runs ago something weird happened. Whenever I enter a room with a safe the safe is already wide open. I can pick up the red gem (and the letter) without entering the code. I'm not sure what caused this. Is it supposed to happen?

A recent patch made it so that any safe you're previously opened will be open when you approach (to save needlessly entering a code you already know for the gem)

If you'd not previously opened those safes then it's a bug.
