I also have reached Room 46, twice in fact, and I recommend reaching it a second time so you can truly visit it.
My thoughts are : this is a Roguelite. As such, "beating the game" takes more than "beating a run".
Compare with Binding of Isaac : beating Mom doesn't mean you've beaten the game, and you could say beating Mom is actually where the "real" game starts.
I believe Room 46 is this game's Mom. Reaching it is only a step in a much longer journey.
These things you have noticed, and so much more that is so far still undiscovered, are all relevant to this longer journey that's still ahead.
That is, of course, if you're curious about what is truly happening.
So really, pull on whatever thread you can!
I'm now in the 40s and still working on things. I've had multiple new puzzles show up I've never seen before, and a couple I'm still not sure where to even begin.
