Isnt the lever Mark can pull what saves him?
There's a whole choice section missing for me.
Here's a video (timestamped it, if its not working @ 3:01:30)

The same happend to me. First and very cheap death.
Thank you for reporting this issue in the "Incinerator" chapter, and I'm very sorry to hear that saving Charlie is proving difficult.
Would it be possible for any of you to provide a video showing that the choices for pulling the grate or trying to open the door are skipped entirely, ultimately resulting in Charlie's death?
We'd really appreciate it.

Can you let me know whether, in your playthroughs where you could not pick any options as Charlie, your partner player solved the wine puzzle and had access to the lever as Mark?
If your partner did have access to the lever and was holding it, but you still could not choose to try the grate or the door as Charlie, can you please let me know? Thank you!
This sounds like an issue with desync that the team are looking into - it isn't related to the issue mentioned in the other posts in this thread, so would be better posted separately to discuss in-depth if need be 😊

The first part sounds like the intended behaviour, as when Jamie, Mark and Kate go into the observation room Charlie is meant to be in the incinerator. So if you weren't already in there, you'd be forced in by the prompt from your partner.
The second behaviour is definitely the intended behaviour, as after checking with the devs, it was confirmed that if you do not solve the wine puzzle before Charlie's scene in the incinerator begins, then Mark cannot use the lever to save Charlie as it remains locked, covered by a guard box. This would be why you cannot choose any option as Charlie in this scenario, as he only gets the options to save himself if Mark is holding the lever to reduce the flames and give him time to come up with a plan (ie. door or grate).
It's as Linkite kindly wrote above:
Having said all this, does this clear up any potential misunderstandings about this sequence for anyone in this thread? In your playthroughs, have you been failing to solve the puzzle and thus not pulling the lever?
Alternatively, I do know of reports whereby the lever is being held by Mark but that isn't detected, so the flames don't die down. If this is what you're experiencing, let me know this as well.
If as many of you could reply again to me and let me know your situations that would be ideal, as I'm wanting to ensure that if there's an issue in this flow, that we know where and what is.
