Apparently you did not replay last boss fight for true ending (Telling Cardinal "Another Way") in which she is alive at end.
Also Forget-Me-Not dlc takes place after true ending (replaying its events before doing the dlc's story, indicating they are going with that ending)

If i am understanding this right, seems some of the announced characters are DLCs and coming at later date (December and April). I feel so annoyed now about the Playable Character trailer making me think they will be in base game
