I've played the game through once, but on replay I seem to have a serious glitch in that one of the flame sites is completely void of the content necessary to progress in the game. There's no quest or ability to obtain a flame that isn't there.

Are you saying that the shrine in Devil's Spine doesn't contain the quest item? Can you please confirm for me that the Fathomless Flame isn't in your storage? Perhaps it's possible that a mis-click sent it straight to your storage by mistake.

When I get to the quest that should be asking for two eels to obtain the flame the entire panel is empty as if it has already been achieved, but I haven't actually done the quest during this play through. it doesn't so much as ask me to complete a task, so it's just an empty shrine.

If I'm understanding you correctly, you're talking about the shrine that's asking for 2 aberrations. This purple grid on the left-hand side has no silhouettes, because you can place ANY 2 aberrations in there! Can you please try doing that and seeing if that works?

That's the one. I placed two aberrations in the box and it gave me the items I needed to progress.
