Creatures that exist in the world and move in real time can attack you during fishing. The night fish that looks like a boat from a distance, the long tunnel monster in gale cliffs, etc.
The random encounters do stop during the minigame, but will hit you even during the day if you are insane (which you often will be if you spent the night fishing). They are based on passing game time and the game will roll one event.
If you fished the whole night to avoid an attack, the game will queue one up for you and send it shortly after you start moving again. You might not even notice it if your insanity level is low, it could be a still twister way in the distance. If your insanity level is high it is usually one of the direct attack ones (or rocks if you are in a low level area).
Yeah I hadn’t had this happen until I made this post, so I guess I was just getting lucky.
