Almost all such quests are "foretold" by the Hearts Charts for those among the 18 Islanders whom we've found—viewed by either pressing L or clicking the Handshake icon on the Bar at the top of Inventory (with Abrill standing facing us).
General rule in most if not all cases: No 3, 6 or 9 Hearts, No Quests...and no more Unlocked Hearts or growing Popularity at the Stall to boost sales (Compendium Tutorials).
There are at least two other types of quest triggers well into game play, one involving two quests together, and one triggered after Abrill walks into a certain part of Barrow End in which she all but gets killed later on.
I'm playing Game 4 following fatal crashes and/or code corruptions during Early Access ...which at least helped me learn my way around this Island (just a bit, though :-)

The only main storyline quest I have left is completing the game itself. Is this the moment you mean when Abrill comes close to death?
