Milker?

In patch 18 update note it says we can purchase a milker from Cassandra. Do you have to have certain lvl of friendship or is it in a special location?

Milker is bought from Rogost, there's a mistake in patchnotes
And when you buy it you use it with left click, not right, bug that according to discord will be fixed in next patch

I can't find it for purchase through either Cassandra or Rogost. Is this something that wouldn't be added to a game already being played? Do I need to start fresh?

Rogost has a wooden stand in front of his carpenter house where he hangs around most of the day. At this stand there are two books. One is for repairs (e.g. the witch stall, your farm well, all the bridges, etc.). The second book contains crafting recipes like the furnace, tools and crafting material like nails. This is also where you can buy the milker (tools section).
