In all cases, what matters is real time spent and using fast-forward doesn't affect the game's clock, so you can use it for killing everyone within X minutes more easily, but it won't help you with keeping an alarm active for 5 minutes.
I didn't swim, just run towards the second gong in normal speed. I guess the 15-second is in-game time, so using fast-forward limit the space for errors.
