Train is Lost Problem
I built a simple train track layout with one bypass loop of about 10 tiles. I have a train station at a coal mine and one at a power station. I used one waypoint so that I could route trains coming from the power station to use the bypass so that loaded trains stay on the original track. I used a one-way block signal at the end of the bypass loop coming back and one at the end just before the original track meets up with the bypass loop near the power station.

The train orders were to go to the mine then to the power station and then to the waypoint before end of orders would send the train back to the mine. I tried two trains on the track and it didn't take long before one of them got lost. This affected both trains and wasn't more than a few trips back and forth before it happened. I'd get one train back on track before the other would get lost.

I don't recall this being such a big problem a few years ago when I first played OTTD. I did check the forum for train is lost posts but there weren't many and one concluded that it's better to just set up separate tracks for each train which shouldn't be the case. With all the fixes over the past few years I assumed that we should be able to use simple bypass loops without trains getting lost. I sure hope this train pathing problem gets some attention by the programmers.

I decided to try making it simpler by eliminating the waypoint and that did the trick! Just using the one-way block signals kept the trains going on the correct path of the bypass loop. Apparently the waypoint only confused the trains. Now they don't get lost.
