Well they're not real. They don't count for anything, they aren't required for 100%, nada. You're not even supposed to know they exist unless you crush a level enough to see one for yourself.
If they were just put on as a fifth tier of medal, completionists of any stripe would feel it necessary to red medal every single level which isn't going to be fun for a lot of them, so best to keep them aside.
Mind you I did actually feel it necessary to red medal every single medal but I am really weird
Totally agree with you in terms of the first playthrough. It wouldn't have been fun shooting for red medals the first time around.
But after beating the game, wouldn't it be cool to unlock the red medals for a second playthrough? Have them operate the same way ace, gold etc. medals do?
It would be another reason to play the game again and improve your times. And think how satisfying it would be to see the red medal appear on screen with the same sounds and images the other medals have.

Yeah, sometimes feels like they aren't that good at their own game :P I got every red medal in ~75 hours of playtime IIRC (done some other stuff since then)
