When they are not in a fleet, they will try to help any ship which got out of fuel. Usually civilian or autonomous escorts.
When they are in a fleet, they will follow the fleet at a distance, refuel them when the ships are idle and go fetch more fuel when their tank is empty (independantly from the fleet's order). Having two tankers in a fleet may allow one to go fetch fuel while the other stays with the fleet.
Note that, for the tanker to refuel the fleet ships, they have to remain static for a while. Refueling takes a lot of time; so while a single tanker is okay for a very small fleet, bigger fleet will need more tankers.
