Are quest reward accessories unique?

Are accessories ex:. "Fly" or "Bar of soap" unique and we can only get them in "bad" choices in one quest?

I believe I've gotten the fly for fishing as random loot in a chest once but that would be relying entirely on RNG. If you are certain you want those items then the quest choices are probably the only guaranteed way to get them.

Once you have decrypted all of the codex in a region, a new tomb will pop up and I have gotten a unique belt from one of them.

I have 2 bars of soap.
Got one early game as random loot and 1 as quest reward.

I think the general answer is "sometimes", but for the two examples you mentioned, the answer is no.

Thanks, The RNG is very stingy. After 100 hours of play and clearing 5 regions i didn't get this two accessories in random loot. So i'm happy that i chose to get "Bar of soap" from quest. Better to check wiki how good is given accessory.
