There are a lot of "things" in the game. Skills that allow you to ignore poison, oils focused on dealing with plague ridden, left hand weapons for various effects, helmet stamps for buffs when debuffed, or buffed, or dealing with dots, etc...
The interesting thing about the arena, is that some battles are easier early, and some are easier later. This seems like something that would be easier later, maybe you haven't found enough "stuff" and the puzzle just isn't possible, because you don't have all the pieces yet.
killing something gave you 1 infection, but some ennemy when killed remove all your infection. (hints => rats)
2nd killing more than 3 don't remove your character right away but only when you end his turn so you can kill more than 3 ans remove all the stack before ending your turn
3rd you kill them with dot like burn and bleed and that don't give infection counter.
If this is true then I'm an idiot: that's fair, I can be one when I'm unquestionably angry. but if that's also the case its not explained. At first glance the game doesn't tell you that there's something to remove infection just that you get 3 and you're out
