If you go against one side the other will hunt you down.
You get also to decide if you poison the water reserves of one of the castles or not.
If you do it, the clan will send a troop (rather easy troop) to hunt you down, you can fight them off and get the goodies.
If you don't decide to do it, you attack the people that ask you to do it and get their goodies.
The goodies differ a little bit depending on what you decide.
The game has several such things for you to decide like: help people that want to live in a unused farm instead of helping the owner, or persuade or bribe the owner.
Decide to kill some infected that are trapped in a cellar over giving the family that has them trapped wood to reinforce the door or food to keep them fed. Whatever you decide a different outcome will take place (either the family dies, the infected die and so on).
Decide if you want to help a guy that sided once with some thiefs and save him from getting killed by the law OR get his thief friends to help him against the law and give him and his wife a free life wherelse.
A LOT of such moral decisions that are fun and help you progress in the game different depending on what you decide.
There are even several recruitable characters that you can only recruit if you pick the right decision.
My tip for you is: choose the free save mode in the beginning and save the game before you pick a decision. If you don't like the outcome just reload and take a other way. ;)
