Well sort off 4 technically missable, 2 are throughout the story, and then another 2 are linked to endings, you can only get 1 of the ending files, not both. So either files save on ur profile and on repeat runs, just get what u missed, or u don't need the 2 ending files.....??

Hoffman's Diary 2, you know when you rotate the stairs and use the plank to cross the gap, u must drop down before using the plank, if not u cannot get the file below it.

Kay's Note, you must give her the Prison Key, then later this will be near where she was standing.

Then there is a file after each endboss from either ending, once you pick up the file, you can't backtrack which is why you can't get both

I have collected all the diaries even when it is time to choose the end, before killing the worm there is one or if you go through the helipad when killing all the zombies there is another but as it is not saved

How are you sure that you have every file, in that case you must think the achievement has bugged.....??

Are you sure the source you're using to check the file quantity is accurate???

I currently have 30 Diary files and 38 Document files, is that the same for you , 68 files in total??
