fertility come from the building (usually) and must be in range, but 1 square should get the bonus (as far as im aware, i have farms that only touch 1 square and they get the bonus)
fertiliser is produced by the building, needing 1/3 water and 2/3 dried manure to make, and once made is taken to storage (you should make a storage right next to the building and dis-allow anything but fertiliser), farmers will come and collect fertiliser to use on the field once they are done sowing (some farms allow more workers than others, more workers means more fertiliser used, so plantations and nurseries may need 2 people to make them pay, later in efficient setups you only need 1).
And the really annoying thing is that the buildings said they'd have the buff when I was placing them. So I know that the game recognises they are within the Compost Plant's range despite having all those agriculture buildings tell me that don't have the buff.
i have found that only the 'farm' parts of farms get bonuses. the nursery in particular has a building at one end, and if that building is the only part in range then the game says you get the buff, but you dont. have to delete the building and make sure the 'farm' part of the nursery is in range to get the buff.
cant say about the dense orchards though...
