I don't understand this game mechanic. I had a 40 yr old city with nearly 300 people, but this city, with Highest happiness and Highest health, produces 0 babies each and every year. It's year 28 and it's been like this for at least 10 years, maybe more. I barely broke 130 people and it's been collapsing rapidly ever since - down to 90 after just 4 years.

Why is this a problem in my city? I'm clearly missing something. Is the mechanic flat out broken?

Also, check citizen ages ... how many child bearing citizens do you have?

In Banished, this issue meant a major die-off was needed before you could recover with child births.

The fertility rate is linked to the following
1: Residents living in low-level apartments cannot have children, but high-level apartments allow children.
2: The lower the happiness, the lower the fertility rate.
3: Adult residents need to move into new housing before giving birth.
4: The elderly cannot bear children.
5: Residents without housing cannot have children.
6: Residents who live alone in a house cannot have children, and must have 1 male + 1 female to have children.

I have this same problem. My population keeps dropping even with steady imigration. 0 children and 0 babies at 100% fertility rate, 50% of housing empty, 60% of population fertile. 500 population down to 200 with no major events.
