For early game, i would says the bells room.

For mid game no idea, im still level 70

In Forsaken Fen from vestige of Valade go through the shortcut door and switch to umbral, climb the ladder to the left and farm all the enemies on your way until you reach the drop to Congregator of flesh boss fight, jump across and reach the first vestige of the area. Repeat until you farm enough vigor and resources.

Any big area can do if you can handle Scarlet shadow. The second Lightreaper arena or Crow arena.

Before judge cleric drop seedling (where you jump to arena) and farm 4x abiding defender, abbess and sanctified hunter in cathedral door.
You get 1-4 large derelium shards a run with eye + ring + rabbit paw. Go umbral always to trigger eye effect. You also get abiding gear, abbess and hunter gear. (really good place to get female fashion souls armors)

Leprosarium vestige kill holy bulwark and rinse and repeat (large shards + holy bulwark gear.) Same go umbral to get that eye bonus.

Skein mines go outside from shortcut door (requires skein mines key) kill inferno enchantress , ruiner and skinstealer great spot to farm inferno gear.

These are places i run for farming.

For vigor, likely farming the first 2 or 3 bosses in NG+1. Not because it's the most vigor possible, but because it's the most per hour without requiring a lot of skill and knowledge of the game. Stay in Umbral with the max multiplier and as many ring, rune, etc... vigor boosts as you can equip and still beat said bosses fast enough to justify equipping them. This was a 2,000,000 per hour farm per the numbers, but I was only doing about 1.5M doing it myself. Possible, isn't the same as doable to be clear.

For runes, that's where ever it's easiest for a player to Soulflay and do whatever the special attack is called when an enemy is put into stagger from the Soulflay when it's health drops to 0. Guaranteed drop of the runes from said enemy in most cases.

As far as items go, that's tricky to answer. Likely doing a vigor farm if they're cheap enough in the shops.

If speaking only of vigor in NG, well, that's basically just running the game in Umbral maxed and focused on bosses back to back fast as well, but, you can run the other vigor farm in Co-Op. Also, I'm speaking of using the reset NG+1 at any time.

Not saying there aren't ways to get things faster, just that these are the most legit ways of getting things. Would also keep in mind farming certain items may be more fruitful for vigor if the drop sells are high enough. I'm not really testing for this anymore. So don't know what the farming is like in the current patch.
