You can still recover health with briostones or Invigorating Aura if you have 22 RAD.
The eye drop and achievement can be done later when the devs decide that uber nerfing is enough...
anyway, my tip is to fight the scarlet shadow in a big open area, the arena where you fight the tutorial boss is a good spot, the umbral monsters are low level there so it's easy to get them out of the way if necessary

2k vigor and five umbral scouring is what you get from beat her second time
Can relaibly kill him this way each time, regardless of what weapon i use:)
Also i only saw him reset to full health when i moved away from him, so as long as you fight him in the area where he spawned, he will never reset.
Oh and dont fight near a vestige. Thats why he despawns and comes back with full health.

Low level area is important to ensure that spawning Umbral enemies will not pose too much of a threat. Healing is important as your flasks are disabled so you need alternate way of healing.
I was farming him both in NG and NG+1 in place in front of Shrine of Adyr-relatively flat area with no obstacles. It is important to also avoid obstacles or disengaging from the fight for too long as the Reaper can reset and heal to full. It does not heal to full anymore unless latest patch broke it again (it was happening after Hexworks buffed him but it was a bug that was eventually fixed).
The fight itself is mechanically rather simple as the Reaper after patch primarily uses submerging attack. This means all you have to do is dodge the attack and punish with dodge-attack. This will make the fight rather slow but safe. If you are durable (high defenses) and have a way to heal, you can try to get few more hits before it submerges again.
