Final achievements won't pop (bugged?)

I still need "None Shall be spared" and "Lingering Moments".

For None Shall be spared I have killed every boss at least twice with the same character. Including the optional ones (kuka, Ebb, Tortured Prisoner, Visage etc)

For Lingering Moments I made an Excel Sheet and have followed this guide:

In the video it even says to do the lightreaper one time in Upper Calrath and one time anywhere else wich I have done (to get the kuka fight to pop) But the achievement still won't pop - no matter what I do :/

In Patch v.1.1.224 the ng+0 mechanic was introduced for those who want to do an "achievement hunt". I am currently in ng+1, but I have probably done around 12 playthroughs as of now (I don't wanna go to NG+2 because no vestiges is a hassle...).

So I am wondering if the Boss/Stigma progress does really carry over if you start a ng+0.
Since I don't know how the achievement implementation with steam works I am unsure if the mechanic was introduced as a "not fully tested hotfix" and if it can impact the trophy...

Anyway I am open for suggestions :/

"Stigma collect" progress carry over to NG0. (proved by myself because I earn this trophy in NG0/3)

Stigma a bound to your character, unless how many playthroughs.

I earned it on my second playtrough on ng+1 (did the Umbral Ending + Drustan's sub-quest, I'm mentioning this one because it gives you 2 more stigmas to see). The achievement poped out after revisiting all the arenas you can fight the Lightreaper (you don't have to kill him before his mandatory fight in Upper Calrath, just revisit the arenas after beating him and in every one there'll be a stigma).
Other than that, it could indeed be a bug.

For the life of me I can't get lingering moments to give me credit. I've tried it on two different characters. I've basically got all the stigmas memorized now from two different youtube videos on the same topic. Haven't found a stigma yet that I don't already know of. I even tried doing the umbral ending twice just because I read someone said their's didn't pop until they killed Elaine the second time. It's the last one I need and have basically given up on it.

The boss fights one was bugged for me too. I had to kill the Tortured Prisoner solo to get credit. My coop kill didn't count even though I was host.

Did one Umbral run yesterday and really got Lingering moments after soulflaying Elianes Stigma - Despite already having bought Eliannes Armor and 2x her weapon @DEVS: wtf?

But I am still stuck on "None shall be spared" - I am sure to have killed all Bosses because I have looked at multiple guides, fextralife, and youtube videos. Is there also some kind of "trick" to get the achievement? Like doing xyz last/again?

I finally got all achievements... I had to defeat the tortured prisoner AGAIN wich I did with her own catalyst in hand (proof that I beat her already before...) My new guess, as to why the achievements didn't pop, is that older save-files for "seen stigmas" or "beaten bosses" were ♥♥♥♥♥♥ through diverse updates...
