Trying to figure out why I didn't get the Wayfarer achievement

So, I've been to every named zone (including Mother's Lull), have activated every beacon and finished the game (with the Umbral Ending), why did I not get this acievement ? Is there some obscure zone one needs to go I didn't find ? Is the place where you fight Adyr a named zone and since I went with the Umbral Ending I dind't get it ? I'd love any help you guys can give me cause I realy don't want to do this trophy on NG+ since the no vestige things make traveling around a complete chore.

can you take a screenshot showing all the vestiges you have? I was able to get this without even trying but I did just run around and visit everything I could by trying to find items.

If you've done the umbral ending the no vestige thing will not even be noticable for you

Did you visit Shrine of Adyr? It's quite simple to miss.
Achievement could be bugged as well.

There seems to be a bug with this achievement. I got it randomly after visiting the castle at the end during my second play through. It seems to just pop when it wants. If it helps, I went for the radiance ending for my first play through and the umbral ending for my second.

If checked with wikis and whatnot, I've been to them all. I pretty much looted every single item in the world, every weapon and armor and important ones at least, might've missed some random smaller item. I also did the dark crusader quest, I’ve been around.

I suppose it is a bug. I'm on a PS5 btw so our bugs may differ, but still it seems this happens to a bunch fo ppl from what I've been reading. It might be that the place you fight Adyr counts and you need to do the Radiance Ending at least once on top of exploring everywhere else, maybe it isn't a bug. That's the theory I'm going with now.

I also finished the game last night with the radiant ending and didnt get the achivement. Ive also been everywhere including mothers lull. I saw that with the inferno ending youre gonna go into the judge clerics mind, maybe this is also counted as a location?

Anybody that got the achivement here, did you go into the clerics mind or did you get it without? Btw im also on PS5

Im currently having the same issue on ps5. This is the very last trophy I need. I've done every ending and even reloaded my save to see if "Inside the Cleric's Mind" would have the pop up that most other locations have. (After several reloads, nothing popped up while there, idk if its bugged or what). I've ran through every other location to ensure the name popped. I'm so disappointed, as I always platinum games that I've purchased and have never experienced a bug like this. Some people say it's the Revelation Depths location, but Ive gotten that to pop up several times. I thought maybe it was the Defiled Sepulcher (the name pops at the spot where you first begin the tutorial) but nope. Not sure what to do, but looking at the 97% on my trophy list is driving me nuts.
