Wiki isn't so great for tracking things as well. It works for spells and umbral eyes but weapons and armor pieces are a complete pain in the butt to collect.
More new people could have bought the game and started it. This would make the number go down because it's no longer 0.2%.
Assuming that's what's going on, though haven't looked into it yet. I got the visited all areas of the game achievement before someone I know that completed every ending. I also haven't gotten a few achievements for things I did in game until well after doing them over and over again. Comment isn't just to you. There's something wrong with the triggers for achievements in the game. Had a few boss fight achievements show up after 4 or 5 restarts after defeating them. Normally right when I enter the game.
Anyway, my only real question about this achievement is do the multiplayer sets count for it? Making good progress on all of them, but that's still going to take a minute if they do.
Much easier to see which sets you are missing pieces from.

I have got the achievement yesterday night, is possible if you do 3 ng+ to have 99% of the armors already. Since the shrine sets are not counted is not really hard.
It's sorted alphabetically while in-game it's sorted by weight >.<
That said you can kinda ignore sets you get from quests, bosses and those you get on the world by collecting item. You need to complete all quests for the platinum/100% achievement anyway so no need to track those, you need to kill all the bosses and get all the remembrances as well so as long as you remember to buy all bosses items you are fine there, and the ones you get from item collecting in the world if you are following a guide or keeping a checklist you will get every item everywhere so those are accounted for too. That leaves you with the enemie drops.
What I did for those was go to a wiki, open the entry on every enemy and then go one by one farming the loot, to make it simpler I grouped them by zones and I'd come up with a good farming route on the zone and gather every armor drop on that area, then move to the next one. Keep in mind some sets have variant pieces in certain slots as well, like the Corrupt Pilgrim Set and the Carrion Knight Set. Lastly there is a very easy to miss piece of chest armor which is the Griefbound Dress, that is a somewhat rare drop from the Griefbound enemies or common ice zombies on the Fief, and ppl tend to ignore those enemies most of the time.
It's kind of a long journey but stick to it and you'll get your trophy. I hope this helped at least a bit and I also hope that if we do get a sequel that they don't do this trophy again, way too many random armor drops from normal enemies, at least give us a good way to track everything if you do this trophy again in a sequel.
