Your Carnal Instinct level doesn't really do anything at this point. There are some dialogue options that are theoretically tied to your CI level, but right now they're just locked/unlocked by default, regardless of your CI level.
Which version are you playing? There is a version that uses Unreal Engine 4 and one that uses UE5, and the content (area, quests, etc., etc.) in the two versions is completely different. The default is the older UE4 version. If you go to the game's Properties in Steam, you can switch to the newer UE5 version on the Betas tab. Note that while the UE5 version is called ue5_beta, it's still actually pre-alpha (like the UE4 version), and the UE4 version actually has more content at this point. For what it's worth, the dialogue options that I mentioned earlier may only be in the UE4 version.
