Explain 'Perfect Switch' Please....

I guess I missed the memo or whatnot during the tutorial. I see quite a few skills offering an increase in damage when Perfect Switch is performed. What exactly is it? How is it performed?
Any and all explanations greatly appreciated in advance.

Antea can perform a combo attack when you switch to her at the perfect moment for certain skills. You must have the skill unlocked to do that.
For example,

Assault - Switch to Antea right after a Front dodge to trigger a Melee attack that ignores resistance.

So basically, move forward, press dodge, and switch characters ('Q' keyboard, 'Y' XBOX or 'Triangle' PS).
or,

Devastating Switch - Switch to Antea right after the third light attack to trigger a Punch attack that ignores resistance and knocks down the targeted enemy.

Light attack three times, then switch characters.
Left click, Left click, Left click, Q
RB, RB, RB, Y
R1, R1, R1, Triangle

I found too much inconsistency on this "perfect switch", i'll do it more easily off combat simply trying to execute it.. than in combat
