The plot is something like this: Steve kills the guys involved in the death of his brother, then goes after the drug trade until he manages to find and confront the Baker, who is the boss of one such drug-dealing empire.

Steve either kills or spares the Baker, then Steve is attacked by Hitmen while relaxing at home. Eventually, an undercover cop infiltrating one of the gangs contacts Steve and offers him information in exchange for killing his boss. Once that is done, the undercover cop reveals that the Accomplice was a drug kingpin that was just using him. The Accomplice used Steve to kill off his rivals in the organized crime underworld, then tried to have Steve killed once he outlived his usefulness. Steve kills the Accomplice and the game ends with him reflecting on how his life turned out.

At some point another criminal underworld bigshot named Empathetic is also involved, but Steve doesn't get to kill him. I kinda forget at what point in the story Empathetic gets involved, too. He gets mentioned a fair bit in the Mercenarism prologue of Intravenous 2.

I might be missing some details but overall, the Accomplice saw that Steve still desired revenge over his brothers death, and he made Steve into a killing machine so he could get rid of his rivals in the drug trade. It backfires horribly.

Also, if Steve spares the Baker, he can continue using the loadout menu on post-Baker confrontation missions. Killing the Baker makes the game harder as you can no longer use loadouts, and have to scavenge for what you can find off any dead bodies.
