Essentially... waves are still triggered by doors, you just have several minutes of grace period in which you can open additional doors when you're ready.

I'm thought I was attacked while reading turret descriptions and trying to decide what to build (which I was doing between waves, not during a wave), but maybe there were enemies that I hadn't cleared from a previous wave or something?
What you described can be the result of one of three things: Like you said you may not have completed the previous wave, if you opened the door to a spawner anything residing there may have come after you, or if you opened any doors after the previous wave another wave could have started. I can't know exactly which it was in your case without more context.
The only completely safe time to read descriptions, pick upgrades, build turrets, go to the bathroom, eat a sandwich, or whatever else you might need, is when the danger indicator by your mini-map reads "Safe". If it says "Danger", you need to be prepared for the next wave (or you can pause in solo play).
