How to unlock more Mutations?
It seems no matter what faction I use, I only get the option of "Who am I"(sp) "Where are we"(sp).

I had only those options when I won by conquest, I got a third when I beat the titans in endgame

That's when you just win a game using alliance victory before the main event (and the question is "What Are You?", not "Who am I"). After the main event on Anchorites side I got:

Emulated Mind:
What Are You?
Where Are We?
Did Our Sacrifice Mean Nothing?

I didn't play anything since then but I would guess you can at least win as Zephon's ally or independent. And then maybe try to do it with different factions.

I completed Fallen Soldier's Meet your Maker questline but didn't get mutators at the end of it. One ending gave achievement, another added one more hp regen per turn. So not all faction questlines end at the mutator unlock it seems.

I got "Did our sacrifice mean nothing?" allying with the anchorite using the untold prophet, got "Why did humanity die alone?" using the emulated mind on independent victory. I imagine this must mean the mutators are tied to endgame alliances and maybe midgame story quests

The Faction Quest needs to be completed, but it needs to be completed with one specific outcome. What outcome that is can be seen when you go to global achievements, spoil the hidden ones and find one with your faction symbol on it. Ignore the one with deep statement, look for the other one.
If that achievement triggers on completing the quest, you are on the right path and will get a third option in the “debriefing”. The fastest and easiest way to get the achievement is playing a two player game with no escalation and number of third party cities zero. Just ally with the other guy once you got the achievement.

What is debriefing? The win screen? Do you need to win the game after finishing the faction quest to get the mutator or should it trigger right after you finish it?

That may explain why I didn't get it. I finished the game by eliminating all factions not in my alliance. Got the two basic question choices. Used One more turn button to continue. Finished faction quest after that. Then waited till turn 100 to see what happens, main event started, my alliance went to the Anchorite's side and I was left alone without being able to join them as I killed them earlier so spoiled my relationship with them, I wasn't prepared to deal with 8 titans and two hordes of other units (or at least remaining 5 after zephon's are crushed).

Hmm. And I decided to use the other option which gave 1 regen instead of the one that gave achievement so even if I continue and somehow win I won't get it anyway.

You need to win. In that weird victory screen where [the god of the voice]? takes everything from you and talks with you. "what are you" and "where are we" are the basic responses that unlock the basic mutators. Finishing the faction quest in the way that gives you an achievement will give you an additional possible response. It doesn't matter how you win, except that all three endgame victories add another possible response. If there is more than one response you want to explore, you can always reload. Since it all happens in the victory screen, I see no way how continuing playing with one more turn could have any chance of unlocking mutators.

I guess there may be a second victory screen after that. I won when I and two other AI players in alliance killed everyone else on the map around turn 60. After that I waited till turn 100 until main event started and new conflict appeared. I expect there may be a second win screen after you defeat all the titans (but I don't have enough forces to actually do this, just killing titans would be easy but hordes of enemy infantry from two factions that left my alliance covering them makes it impossible for my skill level).

You get 2 from beating the game, 1 from each ending, and 1 from each faction quest.
For the faction quests though, you need achieve a specific outcome, and can't just do the quest.
As an example, the earlier post about the Fallen Soldier quest didn't unlock the ending since he chose the wrong option in the last decision.
