The moment I saw the word "colonoscopy" my mind wandered into the dark void trying to find under what circumstance such a word would be used in a game chat!

Chat filters nowadays are more complicated than the word lists of yore. They use machine learning (text classification in this case) to determine whether a phrase is offensive or not. If a phrase is determined offensive/profane/etc, it then calculates a score for each term within the phrase that indicates how much each term contributed to the overall profanity of the phrase. The game then replaces those words with asterisks. It's always possible that some things get filtered that aren't really problematic, but then a certain degree of error is always accepted in any machine learning algorithm.
