How to ge to Cuanacht from Horns of the South??

Jeah..is a transition possible somehow or do i need to start a new game?

there is a portal at the northern edge of HoS where you can go to Cuanacht :)

There are two doors which do it, also you can swim there... which sounds weird but totally works. I only tested it in the western side of horns of the south and only by accident as I was exploring that area.
