We have always played Coop using the In-Game Invite Code and never by an invite through the friend invite system.
Have you tried playing Coop using the invite Code?
This is how we play Coop Sessions with no problems joining:
Host:
Start COOP
Select Game Slot
Create Game (Select Public or Private / Number of Players)
Note: If you do not want other people to join your Session, make sure you make your Session "Private" or Randoms will join.
Start COOP
Host's Game will Load.
Once Host game is loaded, Host selects Options or Menu, depending on which Platform you are on.
At bottom of Menu select "Manage Party" option.
Screen will display "Crossplatform Invite Code" in top left, copy that code and send it to Friends.
Guest/Friend:
Start COOP
Select Game Slot you wish to play from.
Select Connect To Session By Code
Enter the "Crossplatform Invite Code" sent by the Host.
Select Connect
Guest/Friend will join the Host's Session.
myself, also only use co-op albeit PC to PC via steam, we only ever run into 1 single issue.
If the steam cloud save is out of sync with the hosts local save new players get connection failed.
Fix is to get the host to quit back to desktop, wait for the steam cloud to update. Restart the game and it will work fine from then onwards. You may have to repeat this if a third or forth player wanna join after youve been playing a while.
Using this method weve been able to get 4 player games working totally sweet with only 2 attempts. The one that fails and the one that works after doing the above.
ALL our sessions are always private.
As far as a Host having to restart a Coop before a friend can rejoin after dropping out is concerned... This I do know:
Once a person leaves a "Public" Coop Session, they cannot rejoin that same Coop Session. This is not a bug, it has always been that way. In order to rejoin, the Host must restart a Coop Session.
If the Host has the Session set for "Private," then someone who drops out should be able to rejoin without the Host having to restart the Coop Session. They should be able to rejoin using the same In-Game Invite Code. The Invite Code remains the same.
I have always been able to rejoin Hosted Private games after leaving for a break or lost connection as long as the Host remains in game, without the Host having to restart. If the connection is lost because Host loses connection and drops out, then obviously, they have to restart the game.
Which is another good reason for Host's to make sure they remember to set their Hosted Coop to "Private" and not Public if they don't want random players joining their Coop game.
I think they presume everyone will be using discord for that lol
that's why a text chat is needed to find random players in discord
to clarifiy :
1. i dont use any mods, neither my friends
2. all the games were CO-OP Private games and this is still happening
3. with gamecode was happening less but still had to restart the game from time to time
4. the problem is now that even if i restart the game and give my friends the code, or il manualy invite them we still cannot join others games we fail to connect 8 from 10 games
This is going to be a stupid question - but do you have crossplay enabled?
Yes i have, otherwise i cound not generate invite code
That's pretty strange since me and my friend have been co-op'ing for a while and we never had any problems joining his session, albeit both of us run vanilla versions of the game with no mods

if issues are crossplatform then its most likely the mods.
Again make the host start a fresh session after all friends have the mods installed.
This means the host quits to desktop as simply quitting to the menu will not work!
QTD will trigger the cloud update. QTM will not.
more than that i cant help.
We play with mods and it all works fine once everyone has the HOSTS mods installed and the hosts steam cloud save is updated.
edit to clarify further : if Im gonna try a new mod I will msg my friends how to find it in browser and tell them to install it before trying to connect.
