Grandmas become sorta evil and scary looking, 'Wrinkers' begin to spawn, and you have a chance at generating 'wrath' cookies. There are also additional upgrades that continue to unlock along that route.
The grandma changes are purely visual.
Wrath cookies are like a reverse of golden cookies; they have negative effects such as instantly losing a % of your cookers, or reducing your cookie production by a % for a certain amount of time. They also have a chance at 'Elder's Wrath' which increasing all cookie production by x666 for a certain amount of time (a huuuge boost).
Wrinkler mechanic is a tad interesting in that, these things called 'wrinklers' start eating your big cookie. Each wrinkler reduces your CPS (cookies per second) by 5% per wrinkler, but these cookies are stored inside the wrinkler at I think a rate of 1.1 or so (basically generating 10% additional cookies per wrinkler). They can be popped at any time by clicking them a bunch to get those cookies back. Wrinklers are sorta 'core' to the afk gameplay style, where you essentially leave a bunch of wrinklers around your cookie to generate more over time. There are a few upgrades that increase the effectiveness that you'll unlock with ascension points down the line.
Further down the upgrade route you'll eventually unlock the 'Elder Covenant'. This basically lets you reduce your overall CPS by 5% but you basically put an end to all the side effects caused by the evil grandmas for as long as its active. Alternatively you also get the 'Elder Pledge' ability which is to spend cookies for a temporary (30 minutes, can be later upgraded to an hour after pledging like... 10 or 20 times) negation to the negative effects without having to sacrifice the overall 5%.
It's worth progressing the tree for sure as there's a 4% and 5% cookie upgrade available and some crazy boosts for the grandmas production in general, and then if you don't like it just use the pact or covenant.
