Highest rank is Undisputed although it's unclear to me what they're actually measuring. A lot of the Undisputed ranks are achieved by known hackers, though.
I’m Platinum 1 and I’ve beaten a good few Undisputed guys and most of them were much easier than expected.
Well the system never got fleshed out like SF6 or other games. The last known issues that you are describing would happen if two players are evenly matched. I stopped ranked matches a long time ago so it may be different. However I've heard people would lose elo based on having a better fighter vs worse fighter at times too. So it's pretty broken IMO but the idea is good in that if you win by using lesser fighters you can get more points and people using better fighters could get less points or even lose depending on the match up. In reality only so much success can be had using a low rated fighter and most matches end with nearly no points awarded.
